If you are an established patient and have questions regarding your billing or insurance policy, please fill out the box and we will respond to you within 1-2 business days.
We accept nearly all insurance plans including AHCCCS plans (Mercy Care, UHC Community Plan, Healthchoice etc) . We also accept most PPO and indemnity plans. We are also part of the Arizona Care Network (ACN), Phoenix Children’s Care Network (PCCN) and Honor Health/Integrative Care Partners plan. Our goal was to be accessible and create access for all patients in our community.
We accept all major credit cards, ATM/Debit cards and personal checks. We do not accept cash at this time. For your convenience, we offer payment plans through a third party to help ease the financial burden of medical expenses.
Allergy testing is a procedure that is applied toward your deductible. Like any medical procedure, you would be required to meet your deductible before insurance covers the remaining portion. It is your responsibility to know your deductible and how much of this you have already met. You are responsible for these fees after allergy testing has been performed. If you have questions with regard to your deductible and allergy testing fees, we will be more than happy to provide an estimate in advance of your office visit and testing. However, it is your responsibility to request this information in advance.
For patients who are not insured or prefer to pay for our services via cash (out of pocket), we require that you obtain an estimate over the phone prior to an office visit being scheduled. We will provide a quote for our services over the phone. If you agree to the quote, you will be scheduled for an office visit. You will be required to pay for these services upon checking in to your office visit on the day you are seen. We accept all major credit cards, ATM//Debit cards and personal checks. We do not accept cash.
